New Patients:
We thank you for your interest in becoming a patient at Madison Primary Care. Please use the link below to request a phone call from a MPC team member. Proceeding to the link acknowledges the patient's understanding and agreement to the terms outlined below. These terms are detailed in the MPC Office Policies & Procedures, which must be consented to in order to become an established patient with MPC.
- New patients will be contacted by our office.
- We ask that patients please be familiar with the terms of their insurance plan. All services rendered are ultimately the financial responsibility of the patient.
- All co-pays or deductibles will be collected prior to patient office visit.
- Any high deductible policies with remaining patient responsibility will require advanced payment at the time of scheduling, insurance payers do not provide payment on any claims until the patient has met what they determine to be their patient responsibility.
- Some insurance carriers require a Primary Care Provider selection, please designate Dr. Bodla as your PCP prior to your appointment.
- Insurance claims will be filed on behalf of the patient. If a patient has a deductible, or the payer deems the visit as a non-covered or out-of-network service, the patient will be responsible for the balance.
- Any questions or problems with patient insurance should be directed to the patient's individual carrier.
- It is the patient's responsibility to notify our office staff of any insurance restrictions so we can facilitate these restrictions, i.e. lab, imaging, or specialty referrals.
- A $50 deposit will be collected at the time of scheduling, and is applied in the event the prospective patient is a No-Show for their appointment, or does not provide a 48-hour cancellation notice. The fee will be refunded, or applied to the patient's copay or deductible as applicable.
- A credit card on file is required for all new patients, details on this process are contained in our office policies.
- Patients will receive guidance on completing practice consents, demographics, and medical questionnaires via the patient portal. We request these be completed at least 72 hours prior to the appointment.
- We require all New Patients to arrive 20 minutes prior to their appointment to complete the registration process.
